Eligibility

Registration of death occurred to Sri Lankan passport holder/ dual citizenship holder at Sweden, Denmark, Estonia, Latvia or Lithuania can be registered at this Embassy. The declaration should be submitted within three (3) months of such death to the relevant Diplomatic/Consular officer of the Embassy. If not registered, after three (3) months and within one year the Diplomatic/Consular officer should record the reason for the delay and affect the registration. After one year, if the registration has not done, it should get the consent of the Registrar General Department to register the death.

 

The required documents for the registration of a Death

  1. Declaration form filled by the declarant, Documents to prove the identification of the declarant. (Download Form CR2 here)
  2. Submit a document to prove the deceased is a citizen of Sri Lanka. (e.g.- Valid Sri Lanka Passport together with current visa, Sri Lanka Dual Citizenship certificate etc)
  3. Death certificate issued by the relevant country /Hospital records of deceased. (English translation of the Death Certificate attested by the Foreign Ministry of the issued country must attached if the document issued in a language other than Sinhala, Tamil or English)
  4. If the cause of the death is not included in Death certificate issued by the relevant country, it should be proved by supporting documents.
  5. Birth certificate of the deceased.
  6. Cargo receipts will be needed, if the death is been registered in Sri Lanka
  7. Documents to prove the identification of the declarant.

 

Who eligible to apply for registration of a death

  1. The nearest relative of the deceased who was present at the time of the death or the person in attendance to the last illness of the deceased.
  2. A relative of the deceased who resides in the country where the death occurred.
  3. If there is no such relative or any other person who was present at the death, the main occupant of the house where the death occurred.
  4. The officer in charge of the hospital or public institution where the death occurred.
  5. If there are no such persons, the person who takes action to dispose the body of the deceased by burial, cremation or any other means.

Submission of documents

  • If a death occurred in Sweden, Denmark, Estonia, Latvia or Lithuania, declaration for registration of a death should be forwarded to the Embassy of Sri Lanka in Sweden, Stockholm.

You may submit the application to the Consular Division of the Embassy by hand or post. Consular Division open from Monday to Friday except holidays (09.00 a.m. – 01.00 p.m.). Please obtain an online appointment from embassy website (http://stockholm.embassy.gov.lk) prior to visit to the Embassy to ensure the priority service.

Getting Service through post:

  1. All the original documents / applications must attach and applicant must sign in front of the Honorary Consul appointed by the Government of Sri Lanka for the particular country or region. Applicant must ensure that the Honorary Consul signed at the relevant places of all the documents / Applications.

 

  1. Relevant fee must pay to the Embassy bank account (Details given below) and attach the receipt of the payment to the application / Document.

Register the Death at Central Record Room

If it is not been registered and If the declarant is living in Sri Lanka, declaration for registration of a death should be forwarded to the Central Record Room of the Registrar General’s Department, Maligawatte, Colombo 10. (Prior to visit to the Central Record Room, applicants are advised to visit to the Consular Affairs Division of the Ministry of Foreign Affairs of Sri Lanka located at 2nd Floor of the Ceylinco Building, Colombo 01, to pay the fee. Central record room and Consular Division is open from 9.00 a.m. to 3.00 p.m. Except for Saturdays, Sundays and public holidays)

 

Fees:

Swedish Kroner 231 should be paid for registration and issuance of death certificate to the relevant Embassy of Sri Lanka in Sweden.

If the declaration is submitted to the Central Record Room of Registrar General Department, Rs.2170 should be paid to the Consular Division of the Ministry of Foreign Affairs and have to hand over the receipt to the central record room.

IMPORTANT:

  • The payments must be made on the confirmation of application.
  • The Embassy does not accept PAYMENTS MADE BY CHEQUES. If paid by the Debit/Credit Cards (Only accept VISA and Master cards), Bank commission of 1.5% will be added to the payment.
  • Please note that according to the financial regulations of Sri Lanka, the fee paid will NOT be refunded.
